To apply for the Paramedic Membership Program GRANT:
1. Fill out an application.
2. Attach a copy of an identification document (i.e. passport, driver's license, CA issued identification card, etc...) showing you are 55 years and older, and a valid La Habra Heights address.
3. Mail or drop off the documents at City Hall, 1245 North Hacienda Road, La Habra Heights, CA 90631.
